What is Vision Insurance?

A health and wellness plan designed to reduce your costs for routine preventive eye care including eye exams and prescription eye wear (eyeglasses and contact lenses).

Plan Rules:

Eligibility: Eligible full-time employees working 20 or more hours per week, spouse, and dependent children (up to age 26)
Coverage through Mutual of Omaha

Claims must be submitted within 90 days of date of service
