What are Health Screening Benefits?
An annual reimbursement for covered members who complete one of the eligible screening procedures on an elected Accident, Critical Illness or Hospital Indemnity plan.

If you or a covered dependent get one of the eligible screenings, you can file a health screening claim form

Once approved, you will receive a check for the wellness benefit amount

The wellness benefit can be submitted annually as long as your critical illness, accident, and hospital indemnity plans are in force
